He could have beheaded a ball boy and it still wouldn't have prompted a caution from that ref. I seriously think he left his cards in the car.

Denying a goal scoring opportunity isn't that clear cut... and if theres any doubt then the ref can't send him off.... besides Dirk said he didn't send him off for handling outside his area... that isn't a sending off

Because he had a change of shirt due to getting blood on it. I remember the reserves playing without names on their shirts. Perhaps it was one of those as a hurried spare.
